Empowering Choices: Exploring the Best types of Breast Cancer Surgery

Lumpectomy (Breast-Conserving Surgery):

Lumpectomy, also known as breast-conserving surgery, involves the removal of the tumor along with a margin of healthy tissue. This surgical approach is typically performed in early-stage breast cancer cases where the tumor is small and localized. A lumpectomy is often followed by radiation therapy to ensure the complete removal of cancer cells. It allows for breast preservation while maintaining cosmetic appearance and function. I had a lumpectomy on June 15, 2023, I will have radiation next.

Mastectomy:

Mastectomy involves the complete removal of the breast tissue, including the nipple and areola. There are several types of mastectomy:

A. Total Mastectomy: This involves the removal of the entire breast tissue but spares the muscles beneath the breast.

B. Modified Radical Mastectomy: This procedure involves the removal of the entire breast tissue, including the underlying chest muscles. Lymph nodes in the armpit (axillary lymph nodes) may also be removed during this surgery.

C. Skin-Sparing Mastectomy: In this procedure, the breast tissue is removed while preserving the breast skin. This allows for immediate breast reconstruction, improving cosmetic outcomes.

D. Nipple-Sparing Mastectomy: This technique preserves the nipple and areola, removing only the breast tissue. It is suitable for select cases and can provide excellent cosmetic results when combined with breast reconstruction.

Sentinel Lymph Node Biopsy:

Sentinel lymph node biopsy is a surgical procedure performed to determine if breast cancer has spread to the nearby lymph nodes. During this procedure, the surgeon identifies and removes the sentinel lymph nodes, which are the first lymph nodes that cancer is likely to reach. The removed nodes are then examined for the presence of cancer cells, aiding in staging and treatment planning.

Axillary Lymph Node Dissection:

Axillary lymph node dissection involves the removal of a larger number of lymph nodes from the armpit (axilla). This procedure may be recommended when cancer has spread to the lymph nodes or when sentinel lymph node biopsy indicates the involvement of multiple nodes. Axillary lymph node dissection helps determine the extent of lymph node involvement and assists in guiding further treatment decisions. I had 2 lymph nodes that were positive for cancer after I had 14 taken out.

Immediate or Delayed Breast Reconstruction:

Breast reconstruction can be performed immediately following a mastectomy or at a later stage, depending on individual circumstances and preferences. Reconstruction options include implant-based reconstruction, autologous tissue reconstruction (using the patient’s own tissue), or a combination of both. Breast reconstruction aims to restore the shape and symmetry of the breast, contributing to physical and emotional well-being.

Conclusion:

The choice of surgical treatment for breast cancer depends on various factors, including the stage of cancer, tumor characteristics, and individual preferences. Lumpectomy offers breast preservation in early-stage cases, while mastectomy provides a more extensive removal of breast tissue. Sentinel lymph node biopsy and axillary lymph node dissection aid in staging and guide further treatment decisions. Additionally, breast reconstruction offers an opportunity for physical and emotional healing.
